Complex Needs

We deliver services to some of the most marginalised and disadvantaged people in our communities. Many have multiple complex needs. People are referred to HCC either when they are homeless, have mental health problems, substance misuse issues, domestic violence, physical health needs or present other complex needs. Many will have experienced complex trauma and abuse, whilst others may have histories of rough sleeping, relationship breakdown or offending.

Wherever people may have come from and whatever their history, we assess, support and work with each person as an individual. We implement a support plan and risk assessment, enabling service users to take control and move forward with their lives. We support people on a case-by-case basis, this enables us to build a holistic pathway care plan

Keyworkers

Our experienced and highly trained staff are at hands 24/7 providing Support, Care and Advice.

Mediation

Our qualified mediation officers provide support to families and individuals find solutions to issues before they transpire to bigger problems. By listening, being there to offer mediation and advocacy, we recommend solutions to prevent family breakdowns and support young people and vulnerable adults.

Floating Support

Our highly skilled staff provide practical support for all.
This is available for young vulnerable people aged 16-25 who are in receipt of care by the local authority as well as vulnerable adults and older people with complex needs.

Our key workers provide practical assistance, advice, support, and care. Our qualified nurses, counselling therapists and social workers also assist with more specific matters.
The support offered includes:

Young People aged 16-24

  • Support with welfare benefit applications
  • Attendance to appointments offering advocacy and support
    College/training applications
  • Budgeting/money management
  • Managing/sustaining tenancies
  • Interview preparation
  • Medication support

Vulnerable Adults & Older People 55+:

  • Cleaning/domestic chores
  • Cooking/meal preparation
  • Support with medication (from qualified staff)
  • Support completing application forms (Welfare/housing benefits/council tax/social services forms etc.)
  • Attendance to appointments/social groups
  • We support those with specific needs such as people discharged from hospital with complex needs such as stroke, mental health issues, diabetes and more.

Mental Health

HCC offers a range of services tailored to support an individual in their own home who has mental health ailments. The service facilitates additional support with day-to-day household tasks, personal care or any other activity that allows them to maintain their quality of life and independent living with a support package created around the individual needs and routines.

Physical Needs

We provide expert tailored support and care for people aged 18 or over with mild to moderate disabilities, including:

 

  • Physical disabilities-visual impairment, speech impairment & Sensory impairment
  • Behavioural challenges- Autism (ADHD)
